Major Responsibilities:
- Leads and coordinates antimicrobial stewardship initiatives to optimize antimicrobial use, improve patient outcomes, and reduce resistance.
- Evaluates microbiologic and pharmacologic data to design evidence-based antimicrobial regimens tailored to individual patient needs.
- Collaborates with infectious disease providers, microbiology, infection prevention, and other healthcare professionals to develop and implement antimicrobial guidelines, protocols, and order sets.
- Monitors antimicrobial prescribing trends, resistance patterns, and clinical outcomes to identify opportunities for improvement and education.
- Provides education and training to healthcare professionals on antimicrobial stewardship principles, appropriate antimicrobial use, and emerging resistance threats.
- Reviews and interprets culture and sensitivity reports to guide appropriate antimicrobial therapy.
- Participates in the development and maintenance of antimicrobial stewardship metrics and reporting tools, including regulatory compliance with CDC, Joint Commission, and CMS standards.
- Supports formulary management by evaluating antimicrobial agents for inclusion, restriction, or removal based on efficacy, safety, cost, and resistance data.
- Participates in the on-call prior authorization program, rotating one week out of every five. On-call hours are from 7:00 AM to 11:00 PM.
- Oversees pharmaceutical care, including retrieving and evaluating biomedical literature and designing medication therapy regimens.
- Uses effective patient education techniques to counsel patients and caregivers on antimicrobial therapy, adverse effects, compliance, and administration.
- Practices in compliance with organizational policies and procedures, as well as local, state, and federal regulations.
- Serves as a preceptor and mentor for pharmacy students, residents, and healthcare learners in antimicrobial stewardship and infectious diseases.
- Participates in medical emergencies and infection-related outbreak responses when applicable.
- Supports department medication safety and quality improvement activities, including reporting adverse drug events and antimicrobial-related issues.
- Directs and checks the accuracy of the work of pharmacy support team members to ensure safe and efficient workflows and regulatory compliance.
Licensure, Registration, and/or Certification Required:
- Licensed in the state(s) of practice or eligible for reciprocity required.
- Board certification in Infectious Diseases Pharmacy (BCIDP) or Pharmacotherapy (BCPS) preferred
- Credentialing and privileging required based on area of practice.
Education Required:
- Doctorate degree in pharmacy from an ACPE-accredited school of pharmacy required, or an FPGEC certificate from NABP.
- PGY1 pharmacy residency training or equivalent
- PGY2 infectious disease pharmacy residency strongly preferred
- Board certification (BCIDP) preferred
- Current pharmacist license in North Carolina or ability to obtain
Experience Required:
- Minimum of 3 years of experience as a licensed pharmacist, including residency.
- PGY1 residency required; PGY2 residency in Infectious Diseases or equivalent experience strongly preferred.
- Experience in antimicrobial stewardship program development or participation required.
- Direct experience with training, education, or preceptor roles required.
Knowledge, Skills & Abilities Required:
- Advanced knowledge of infectious diseases, microbiology, pharmacokinetics/pharmacodynamics, and antimicrobial resistance.
- Strong analytical and data interpretation skills for stewardship metrics and clinical decision-making.
- Excellent communication, collaboration, and presentation skills.
- Proficiency in clinical decision support tools and electronic health records.
- Demonstrates leadership, team building, and conflict resolution skills.
